At least one of your private lessons will be made available to live auditors. Auditors will only be admitted up to five minutes prior to the start of the lesson. Once the lesson starts, auditors will not be admitted. If you agree to allow auditors into your lessons, please check the "Yes" box below.
Participant Consent and Release Form

In consideration of my participation, I hereby grant permission to the Clarinet Maestro Festival (CMF) to use my appearance in connection with the CMF for publicity, advertising and promotional purposes. This includes my image, and/or voice, as well as any results thereof in connection with the CMF.
I hereby authorize the CMF to photograph and record lessons, master classes, workshops, open forums and student recitals in which I will participate in. We may ask for individuals permission to use videos of their playing for marketing, grant funding proposals, fundraising purposes and publicity.
I agree not to record any classes on any external device including, but not limited to, a cell phone, computer or camera. If these rules are violated, we have the right to contact you to remove the recording or pursue legal action if not immediately corrected. Recordings of each class will be given out on a case-by-case basis. If given a recording of a class or lesson, participants are forbidden from downloading, publishing or sharing any class to the internet or distributing the recording in any manner. Recordings will be accessible up to one week after the festival ends.
I expressly release CMF from and against any and all claims which I have or may have for invasion of privacy, defamation, or any other cause of action arising out of my participation in CMF.
